Myoglobinisacytoplasmichemoprotein,expressedsolelyincardiacmyocytesandoxidativeskeletalmusclefibers,thatreversIBLybindsO2byitsheme(iron-containingporphyrin)prostheticgroupinthecenteraroundwhichtheremainingapoproteinfolds.Theredcolorthatmeattakesispartlydeterminedbythechargeoftheironatominmyoglobinandtheoxygenattachedtoit.Whenmeatisinitsrawstate,theironatomhasachargeof+2andisboundtoanoxygenmolecule.Myoglobinisasingle-chainglobularproteinof153aminoacidsandhasamolecularweightof16,700daltons.Unliketheblood-bornehemoglobin,towhichitisstructurallyrelated,thisproteindoesnotexhibitcooperativebindingofoxygen,sincepositivecooperaivityisapropertyofmultimeric/oligomericproteinsonly.Recentgenedisruptionstudiesusingthemyoglobindeficientmousemodelsupporttheconclusionthatthefunctionsofmyoglobininclude:facilitatedoxygentransport,thestorageofoxygenandascavengerofnitricoxideorreactiveoxygenspecies.Myoglobinisanearlyindicatorofcardiacischemiathatachievesmaximalsensitivity3to4hoursaftersymptomonset.However,itisonlymarginallymoresensitivethanCK-MBearlyaftertheonsetofsymptoms,anditislesssensitive6to8hoursaftersymptomonset.Becauseofrapidclearancefromtheblood,myoglobinmay"miss"late-presentingpatients,anditislesscardiospecificthanCK-MB.
