Creatinekinase(CK),alsoknownasphosphocreatinekinaseorcreatinephosphokinase(CPK)isanenzymeexpressedbyvarioustissuetypes.ItcatalyzesthereversIBLetransferoftheN-phosphorylgroupfromphosphocreatine(PCr)toADPtoregenerateATP.Creatinekinaseplaysakeyroleintheenergyhomeostasisofcellswithintermittentlyhigh,fluctuatingenergyrequirements,suchasskeletalandcardiacmusclecells,neurons,photoreceptors,spermatozoaandelectrocytes.Creatinekinaseconsistsoftwosubunits,whichcanbeeitherB(braintype)orM(muscletype).Therefore,threedifferentcytosolicisoenzymesexist:CK-MM,CK-BBandCK-MB.CytosolicCKisoenzymesarealwaysco-expressedinatissue-specificfashiontogetherwithamitochondrialisoform.SkeletalmuscleexpressesCK-MM(98%)andlowlevelsofCK-MB(1%).TheheartmuscleexpressesCK-MMat70%andCK-MBat25-30%.CK-BBisexpressedinalltissuesatlowlevels.CytosolicCKs,incloseconjunctionwithCa2+-pumps,playacrucialrolefortheenergeticsofCa2+-homeostasis.OctamericmitochondrialMi-CKbindsandcrosslinksmitochondrialmembranes.TheCKsystemisregulatedbyAMP-activatedproteinkinaseviaPCr/CrandATP/AMPratios.Thecardiac-specificisoenzymeofcreatinekinase,CK-MB,isabioMarkerformyocardialinfarctionalongwithothermarkerssuchascardiacTroponinIandmyoglobin.TheintroductionofimmunologicmassdeterminationofCK-MBwasamajorbreakthroughthatreplacedthetrADItionalenzymaticassay.
