Tumornecrosisfactor(TNF)-αisanimportantcytokineintheinnateimmuneresponseproducedbymacrophages,neutrophils,fibroblasts,keratinocytes,NKcells,TandBcells,andtumourcells.TNF-αmediateshostresponsesinacuteandchronicinflammatoryconditions,andisamediatorofprotectionfrominfectionandmalignancy.TNF-αcausesapoptoticcelldeath,cellularproliferation,differentiation,inflammation,tumOrigenesis,andviralreplication.TNF-αisinitiallyproducedasaBIOLOGicallyactive26kDatransmembraneprotein,whichissubsequentlycleaved,principallybyTNF-α-convertingenzyme(TACE),toreleasethe17kDafreeprotein.TheseproteinsarearrangedinbiologicallyactivehomotrimersthatactontheubiquitouslyexpressedTNF-αreceptors1and2(TNFR1andTNFR2).These17kDaTNFprotomersarecomposedoftwoantiparallelβ-pleatedsheetswithantiparallelβ-strands,forminga"jellyroll"β-structure,typicalfortheTNFfamily.TNF-R1isconstitutivelyexpressedinmosttissues,andcanbefullyactivatedbyboththemembrane-boundandsolubletrimericformsofTNF,whileTNF-R2isonlyfoundincellsoftheimmunesystemandrespondtothemembrane-boundformoftheTNFhomotrimer.Althoughtheirextracellulardomainssharestructuralandfunctionalhomology,theirintracellulardomainsaredistinct.Tumornecrosisfactorpromotestheinflammatoryresponse,whichinturncausesmanyoftheclinicalproblemsassociatedwithautoimmunedisorderssuchasrheumatoidarthritis,ankylosingspondylitis,Crohn"sdisease,psoriasisandrefractoryasthma.ThesedisordersaresometimestreatedbyusingTNFinhibitorssuchasinfliximab(Remicade)oradalimumab(Humira).CytokinethatbindstoTNFRSF1A/TNFR1andTNFRSF1B/TNFBR.Itismainlysecretedbymacrophagesandcaninducecelldeathofcertaintumorcelllines.Itispotentpyrogencausingfeverbydirectactionorbystimulationofinterleukin-1secretionandisimplicatedintheinductionofcachexia,Undercertainconditionsitcanstimulatecellproliferationandinducecelldifferentiation.
