SildenafilcitrateisanorallyactiveselectiveinhibitorofcGMP-specificPDE5(phosphodiesterasetype5),anenzymethatregulatesbloodflowinthepenisandwhichisthepredominantPDEisoenzymeinhumancorporacavernosa.Sildenafilcitrateisusedasanagenttotreaterectiledysfunctionandpulmonaryarterialhypertension(PAH).Studiesinvitrohaveshownthatsildenafilhasbetween10and10,000-foldgreaterselectivityforPDE5thanforotherphosphodiesteraseisoforms(PDEs1,2,3,4,6,7to11).Inparticular,sildenafilhasgreaterthan4,000-foldselectivityforPDE5overPDE3,thecAMP-specificphosphodiesteraseisoforminvolvedinthecontrolofcardiaccontractility.SildenafilisaweakinhibitorofthecytochromeP450isoforms1A2,2C9,2C19,2D6,2E1and3A4(IC50>150mM).
