CardiacglycosidefromtheleavesofDigitalislanata.Itisusedtotreatcardiacarrhythmias.ItisoftenusedasamodelsubstrateofP-glycoprotein(Pgp)(0.01-10µM)andaninhibitorofNa+/K+-ATPase.Inhibitsmembrane-boundα-subunitsoftheNa+/K+ATPasepumpinmyocytes.TheprimarymechanismofactioninvolvesinhibitionoftheNa+/K+ATPase,mainlyinthemyocardium.Thisinhibitioncausesanincreaseinintracellularsodiumlevels,resultinginareversaloftheactionofthesodium-calciumexchanger,whichcausesanincreaseintheintracellularcalciumconcentration.Thisleadstoadecreaseinheartrateandincreasedstorageofcalciuminthesarcoplasmicreticulum,causingacorrespondingincreaseinthereleaseofcalciumduringeachactionpotential.
