α-fetoprotein(AFP)isaglycoproteinof590aminoacidscontaining3.4%carbohydratecontentwithamolecularweightof61,000–75,000Da.AFPisnormallyproducedinthedevelopingembryoandfetusbythefetalyolksac,thefetalgastrointestinaltract,andeventuallybythefetalliver.Inhumans,AFPlevelsdecreasegraduallyafterbirth,reachingadultlevelsby8to12months.NormaladultAFPlevelsarelowandAFPhasnoknownfunctioninnormaladults.TheBIOLOGicroleofAFPhasnotbeendefinedyet.Becauseofitsbiochemicalsimilaritytoalbumin,ithasbeenpostulatedthatAFPcouldbeacarrierprotein.Itmayhaveanimmunoregulatoryfunctionduringpregnancy.Increasedserumlevelsarefoundinsometumors,suchashepatocellularcarcinoma(HCC),hepatoblastoma,andgermcelltumors.AlthoughtotalAFPisausefulSEROlogicalMarkerfordiagnosisofHCC,thefalse-negativeorpositiveratewithAFPlevelisveryhigh.AFP-L3,anisoformofAFPwhichbindsLensculinarisagglutinin,canbeparticularlyusefulinearlyidentificationofaggressivetumorsassociatedwithHCC.AFPmRNA,thecirculatinggeneticmarkers,alsohasbeenusedinmonitoringdistalmetastasisorpostoperativerecurrenceofHCC.Bindscopper,nickel,andfattyacidsaswellas,andbilirubinlesswellthan,serumalbumin.Onlyasmallpercentage(lessthan2%)ofthehumanAFPshowsestrogen-bindingproperties.
