α1-antitrypsin,52kDa,istheprototypicmemberoftheserineproteaseinhibitor(serpin)superfamily,whichhaveamajorroleininactivatingneutrophilelastaseandotherproteasestomaintainprotease-antiproteasebalance.Theproteinissynthesizedmainlyintheliver,butexertsitsmainfunctioninthelungwhereitinhibitsavarietyofserineproteases,butprimarilyneutrophilelastase.Itprovidesmorethan90%oftheantiproteolyticshieldofthelowerrespiratorytractprotectingtheelastictissuefromdegradation.Disordersoftheenzymeincludeα1-antitrypsindeficiency,ahereditarydisorderinwhichlackofα1-antitrypsinleadstouninhibitedtissuebreakdownduringinflammation.Thiscausespulmonaryemphysemaandleadstolivercirrhosisinseverecases.Inhibitorofserineproteases.Itsprimarytargetiselastase,butitalsohasamoderateaffinityforplasminandthrombin.IrreversIBLyinhibitstrypsin,chymotrypsinandplasminogenactivator.Theaberrantforminhibitsinsulin-inducedNOsynthesisinplatelets,decreasescoagulationtimeandhasproteolyticactivityagainstinsulinandplasmin.ShortpeptidefromAAT(SPAAT)isareversiblechymotrypsininhibitor.Italsoinhibitselastase,butnottrypsin.Itsmajorphysiologicalfunctionistheprotectionofthelowerrespiratorytractagainstproteolyticdestructionbyhumanleukocyteelastase(HLE).
